FINANCIAL INFORMATION.
To our valued patients,
- For your convenience, we accept Visa, MasterCard, and Discover. We deliver the finest care at the most reasonable cost to our patients; therefore, payment is due at the time service is rendered unless other arrangements have been made in advance. - A non-refundable consultation fee will be charged for your initial consultation visit irrespective of whether subsequent treatment is rendered and if you should elect not to proceed with treatment at all.
- Please check with your medical insurance plan regarding any financial coverage you may or may not have for the procedures to be performed.
- We submit a pre-treatment estimate to your dental insurance for you. They usually mail you a response within approximately 2 weeks. You are also able to check these benefits sooner online. If you have not received a response from your insurance company, you can call them directly. For those patients having treatment on the same day as their consultation, no pre-estimate can be sent to your insurance but a guide to the costs of the procedure can be provided to you should you so desire.
- Medical insurance does not accept pre-treatment estimates and because of this we recommend you call your plan directly to ascertain possible coverage or possible costs you may incur.
- Please check if any prior authorization or PCP referrals are required for treatment in the office setting. If your insurance provider requires you to have a PCP referral on record prior to being seen by a specialist, this referral must be in place prior to the patient arriving for their appointment at our office. If your insurance company does not have this PCP referral on record, they will deny the claim. The full balance of this appointment will be the patient’s responsibility.
- If our office is not informed of the need for prior authorization for surgery in the office setting prior to treatment, your insurance company will deny coverage for that service. This will result in full financial responsibility for the cost of treatment falling on you, the patient.
- Please check with your insurance company what their policy is if you are out of network. If we are out of network with your insurance, there may NOT be any coverage at all or there may be a higher patient balance.
- Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
- We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated.
- Any charges not paid are subject to collection and you will be responsible for all costs of collection brought by this office against you, including interest and attorney fees provided by law.
